Appeal
A process that a patient and provider begin to demand that a payer or health plan actually pay for a service that has been denied payment. Also, a special kind of complaint a patient or provider may make if they disagree with certain kinds of decisions made by Medicare, insurers or health plans. Patients can appeal if they request healthcare services, supplies or prescriptions that they think they should be able to get paid for by their health plans.
