Fraud
Intentional misrepresentations that can result in criminal prosecution, civil liability and administrative sanctions. In health care, fraud most commonly refers to hospital and doctors that are suspected of charging fees for services not provided or have, in some other way, incorrectly documented a medical record in such a way to increase revenues or avoid scrutiny.
